FINANCIAL POLICY
Welcome to Prime Health Family and Sports Chiropractic Wellness Center. Our primary mission is to deliver the finest and most comprehensive health care available today. Our experience shows that it is wise to have an understanding with our patients as to our office policies and fees. We offer several methods of payment for your chiropractic care at our office. You may choose the plan which best fits your needs. Please read over the options carefully and choose the plan you prefer. This information enables us to better serve you. Our main concern is your health and well-being.

A. CASH (Includes checks)
1. Payment in full is expected at the time services are rendered.
2. You may make a single payment in full at the beginning of the week for multiple visits during the week.

B. MAJOR CREDIT CARDS (Includes MasterCard, Visa or Discover)
1. Payment in full is expected at the time services are rendered.
2. You may make a single payment in full at the beginning of the week for multiple visits during the week.

C. PROMPT PAYMENT AGREEMENT (For patients with/without insurance)
Medicare, Medicaid, Personal Injury and Worker’s Compensation do not qualify.

• A discount of 30% will be given with payment of Cash or check at the time of service. A discount of 25% will be given with payment by credit card at the time of service. Administrative costs, such as insurance processing, are one of the components used in an equation developed by the Centers for Medicare and Medicaid Services to arrive at reasonable fees physicians may charge for the services they provide. The elimination of this component results in monetary savings shared throughout our industry. We offer you this savings in the form of a courtesy discount to patients or third party payors who MAKE PROMPT PAYMENT AT THE TIME SERVICES ARE RENDERED. Patients/payors electing this payment option understand that discounts are offered only for the elimination of insurance paper work and that this office does not provide patients/payors with superbills, completed claim forms, or written reports. Consult the front desk for full details.

D. INSURANCE
It is important that you understand your insurance policy is a contract between you and your carrier. These days, many policies reimburse for at least some chiropractic care. Coverage varies greatly from policy to policy and constantly changes. Our goal is to help you get well and stay well. Sometimes this is at odds with the profit motives of an insurance company.

This is frustrating for you and us.

YOU are responsible for the full amount of your bill if, for any reason, your insurance claim is denied.

We will contact your insurance company to verify coverage. Services will be provided on a cash basis until verification of coverage can be made. You are responsible for deductible and/or co-payment at the time services are provided.
